If you are insured by a different provider, I will provide an electronic courtesy submission to your insurance carrier. If the insurance does not accept electronic submissions, I will provide a super-bill to you that you can submit to your insurance company. Many insurance companies do not pay for couples or family therapy. However, if there is a mental health issue that is significantly impacting the functionality of the relationship or family, then they may pay for counseling.
Access to Personal Records
Most insurance companies require a mental health diagnosis before they will distribute reimbursements. Because of this, your insurance provider may require a greater level of detail about your presenting problem than you are comfortable with them knowing. In such cases, you may choose to pay for services on a fee-for-service basis. This means that you will pay for services at each session, which removes the need for disclosure to a third party.
Payments & Billing Information
Rate for 45 minutes: $129 ($120 if you pay cash)
Rate for 25 minutes: $70 ($60 if you pay cash)
Rate for 60 minute workshops: $70
I accept cash, checks, pay-pal and credit cards. Payments are due when you book your appointment. The Credit Card Authorization Form will need to be reviewed, initialed as indicated, and emailed to email@example.com prior to seeking services.
Clients will be billed for any unpaid balances. A monthly financial charge of 1.5% is charged for balances exceeding 30 days. Accounts that are more than 90 days overdue may be reported to a collection agency. Please note that there is a $25.00 service charge for returned checks.
